New Patient Paperwork and Forms.

Integrated Movement and Health uses a 3rd party HIPAA compliant program for providing electronic intake and new patient forms.  Once you set up for an appointment with Integrated Movement and Health, you will receive paperwork and forms via email or text message that should be filled out prior to your session in order to allow for your care to be the sole focus of your session.
